Does the provider include the patient, patient’s family and health care partners in the care planning process?
Yes, we believe that it’s a team effort. Care plans are client centric, which means, the care plan is designed to address your desired outcomes with the help of the patient, health care partners and family.
Does your agency offer the services that you require?
KeynCare provides support to clients with CHF (congestive heart failure), COPD (chronic obstructive pulmonary disease), Parkinson’s, Alzheimer’s Dementia, Diabetes, Orthopedic/or Surgery recovery support, and other age related ailments that may require personal care, medical attention and companionship. KeynCare provides specialized training to ensure our caregivers are knowledgeable and well equipped to aid in managing the disease process.
Does your agency provide literature explaining their service in detail?
Yes, we provide educational material that outlines how we help manage the disease process inclusive of all services offered.
Are the services and financial terms in writing prior to the start of care?
Prior to service rendered, the financial terms are discussed and presented in the form of a written contract. Currently, the payor sources accepted are: LTC insurance, Private Pay (out-of-pocket), Medicaid waiver, and Options Waiver.
Do they ensure that an emergency plan is in place?
Upon in take, an emergency plan is created with the patient and family. A copy of the plan remains in the home for our caregivers to reference and a copy is also maintained in our local office.
How are caregivers selected and trained?
Our staff consist of well trained licensed and unlicensed care providers. We employ direct care workers for companion care and CNA’s and HHA’s for clients needing personal care and/or other specialized services prescribe by your health care team. We also have an RN that conducts initial in-home assessments, hands on training and frequent follow-up to ensure the care plan meets the needs of our client. We believe that the role in caregiving requires skill, knowledge, and training this is why we have partnered one of the best online training solutions in the industry.
Does the provider have a process for overseeing the quality of care?
Yes. Our field case manager/RN provides hands-on training to ensure personal care is properly performed. Additionally, KeynCare provides consistent follow up with clients and their family to ensure care needs are meet and exceed expectations. Our supervisory team conducts frequent in-home visits to assess care delivered. We also provide our clients with surveys to evaluate an opportunities to improve the service we provide. Service is our priority.
How are questions or complaints resolved?
Questions or Complaints can be submitted by phone call, email and/or letter. Complaint forms are issued in our Welcome packet upon signing of contract along with detailed instructions. Concerns are addressed immediately with safety as our top priority.
Does the provider document tasks performed and course of treatment rendered?
Our team completes a task sheet daily. This task sheet is kept in the home and submitted to our office weekly. Our caregivers also submits an electronic task sheet for our immediate review.
